NEW PATIENTS
Please fill out our online forms so we can expedite your first visit
NOTE: Not a great typist & or have too many medications? Just bring your medication list both prescription(RX) and over-the-counter (OTC) to your appointment and we'll enter them for you.
CURRENT PATIENTS
Please plan to come 15 minutes before your appointment time and we'll have you review your information on a tablet.
What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. We look forward to your first visit.
Appointments
If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
Payment is due on the day of service. We submit electronic claims on our patients’ behalf to most traditional plans and currently participate in Delta Dental Premier and Horizon BCBS of NJ (Traditional) (subject to change). For all other plans we are “out-of-network”. We will estimate the patient portion which is payable on day of service and we will balance bill if your insurance pays less than expected. If you are unsure about your dental benefits, please call your insurance company PRIOR to your appointment to see if you have “Out of Network” benefits. We also offer flexible payment plans through CareCredit, please ask Joanne or Dawn for details.
Financing Options
We do not have in-office payment plans, but CareCredit can help you pay for dental treatment needed now with their flexible monthly payment plans.
